Patient Responsibilities


We ask that you treat all practice staff with courtesy and respect.

You are responsible for your own health and that of your children.

We will give our professional help and advice, please act upon it

Let us know immediately if you change your address or name and remember to give your phone number and postcode.

Please contact the surgery between 8.00am and 10.00am for a home visit during the day.

If tests are ordered for you, please ask your doctor or a member of staff about receiving the results.

Arrive on time for your appointment. Patients who are late for their appointment may not be seen or may be required to wait until the doctor is free.

Be patient if we are running late

Do not expect a prescription every time you visit the GP.

Patients should only request an urgent appointment if appropriate

An appointment is for one person only - where another member of the family needs to be seen or discussed, another appointment should be made.

Please let us know if you are unable to keep an appointment. We can then offer this appointment to someone else.

Where an appointment or acknowledgement of a routine referral for a hospital appointment is not received within six weeks, you must contact the booking office concerned.

Only request out-of-hours calls (e.g. evenings, nights and weekends) only if they are felt to be truly necessary.
